Please fill out your information, as well as the info on the store you are planning to visit.  Once you submit the form, we will give you a special code that you can print out or screen shot to send to the store on your visit.   Once the store places an order, we will contact you for the info on the custom logo we will make for you.  In addition, we will send you a custom care package with some of our products, which may or may not include WHAMblems, stands, or other products.
